The Emergency Notification Additions screen lets you add the employee emergency notification information. Information maintained includes: doctor's name and phone numbers (2), as well as primary and secondary next of kin demographics information (name, address, phone numbers (2), primary and secondary languages, religion, employer, and supervisor.
Employee
Enter the Employee Number this Emergency Notification relates to. Search to display a list current employee's.
Doctor's Name
Enter the name of the employee's doctor. The entry screen used for this field is identical the one used in Employee Additions.
Last Name
Enter the Doctor's last name. 25 characters are allowed.
First Name
Enter the Doctor's first name. 15 characters are allowed.
Middle Name
Enter the Doctor's middle name or initial. 10 characters are allowed.
Doctor's Phone #1
Enter the Doctor's phone number. (Suggest using this field for the office number. Enter the number without '(' or '-'. The system reformats the number.
Doctor's Phone #2
Enter the Doctor's phone number. (Suggest using this field for the doctor's answering service/pager. Enter without '(' or ''. The system reformats the number.
Next of Kin Screen:
Dependent
Enter an employee's dependent number, leave blank if the primary Next of Kin is not a dependent. Search to display all dependents for the employee.

Salutation
Enter the salutation to use in correspondence with this person. Typical entries are Mr., Miss, Ms, Dr. This field may be left blank if desired.
Name
Enter the persons name. A window will open for entry of the name data.
Last Name
Enter the persons last name, 25 characters are allowed for this field.
First Name
Enter the persons first name, 15 characters are allowed.
Middle Name
Enter the persons middle name or initial, 10 characters are allowed.
Home Phone
Enter the home phone number. Do not place ( ) around the area code or a ' - ' between the sections. The system reformats and displays the number. This field can be left blank if unknown.
Work Phone
Enter the persons work number. Do not place brackets ( ) around the area code or a '-' between sections. The system reformats and displays the number. This field can be left blank if unknown.
Primary Language
Enter a Primary Language Code from the database. Search will display all valid codes. This field is optional.
Secondary Language
Enter a Secondary Language Code from the database. Search to display all valid codes. This field is optional.
Religion
Enter the persons religious affiliation if any. 3 characters are provided.
Address 1
Address 2
Enter up to 30 characters for the street address. If mail is delivered to an address other than their street address, enter the mailing address. This field can be left blank if unknown. Use field 10 to complete the address if required.
City
Enter the city the person lives, or where the mailing address is located. This field can be left blank if unknown.
Province
Enter the 2 character province code where the person lives, or where the mailing address is located. This field is validated against a table containing information taken from the Revenue Canada T4 information book. Leave blank if unknown.
Postal Code
Enter the postal code for the above address. This field can be left blank if unknown.
Country
Enter the 3 character code for the country above address is located in. This field can be left blank if the information is not known.
Employer
Enter the Primary Next of Kin's employer. This is a 30 character text field.
Supervisor Name
Enter the Primary Next of Kin's Supervisor's name. This is a 30 character field.
