The Evaluation Additions module lets you add employee evaluations.
Employee
-
Enter the number of the employee to access. Search for valid employees.
Position
-
Select the position this employee is currently assigned to from the drop down. If the employee has assignments they will populate in a list. If the employee does not have an assignment the Position Code Database will populate in list form to make a selection.
Assign Start

The assignment start date will automatically populate when the position is selected. If no assignment you can manually enter the date the employee's assignment starts as YYYYMMDD or YYMMDD.
Evaluation Code

Enter a valid evaluation code. Search to view valid evaluations.
Location
-
Enter the code of the location where this employee works. Search to view valid location codes. This field is optional.
Required Date
-
Enter the date the evaluation is required as YYYYMMDD or YYMMDD. This date is calculated for you if you set up the Required Date Source and Required Date Interval fields on the evaluation code. This date will be the number of days (as determined by the interval code) after the assign start date or hire date depending on whether you enter 'A' or 'H' in the source prompt. This date will also be used as the base for determining the notification due dates.
Completed Date

Enter the date the evaluation was completed as YYYYMMDD or YYMMDD.
Completion Code

Enter the completion code that indicates the completion status of the evaluation. Search to view valid completion codes.
Completed By

Enter the number of the employee who completed the evaluation. Search to view valid employees.
Supervisor

Enter the employee number of the employee's supervisor. This is the person to whom the notification form will be directed. Search to view employees.
1ST Interval

Enter the interval code to determine when the 1st evaluation notification should be sent to the employee. The notification date will be calculated by subtracting the interval days from the evaluation required date. Search for valid interval codes.

This is the date the first evaluation notification is due to be sent. It is calculated automatically if you enter an interval code. Enter as YYYYMMDD or YYMMDD.
1ST Date Sent

Enter the date the first notification form was sent as YYYYMMDD or YYMMDD.
2ND Interval

Enter the interval code that determines when the 2nd evaluation notification should be sent to the employee. The notification date will be calculated by subtracting the interval days from the evaluation required date. Search to view valid interval codes. If you are not interested in sending out notifications leave this field blank.
2ND Date To Be Sent

This is the date the second evaluation notification is due to be sent. It will be calculated automatically if you enter an interval code. Enter as YYYYMMDD or YYMMDD.
2ND Date Sent

Enter the date the second notification form was sent as YYYYMMDD or YYMMDD.
Final Date Sent

Enter the date the final notification form was sent as YYYYMMDD or YYMMDD.
