The Employee/Dependent Benefit Report lets you view current and historical benefit information. This report shows all the information required by ASEBP. An impressive selection screen lets you choose what records to view or print.
As Of Date Range

This prompt lets you select whether you want to see current or historical benefit information. You are required to enter two dates to create a date range. All records in the date range will be reported. If you want just the current records, enter today's date in both the From and To columns.
Employee Categories
Employee Groups

Employee Status

Enter two dates ((YY)YYMMDD) that make up a date range. Only employees with a Leave OF Absence Date between the specified range will be selected.
Employee Salary

Enter two dollar amounts to make up a dollar amount range. Only employees with a salary between the specified range will be selected.
Benefit Types

Enter 'D' to select only Dental benefit information (employee and dependent(s)).
