COVID 19 Provincial Attestation Form and Report

Providing a quick entry form and administrative report to support the provincial mandate requiring staff to report their vaccination status.

Why we think you’ll like it:

To assist districts with compliance for Provincial COVID 19 Reporting, the new Attestation form provides a quick way for employees to enter their vaccination status, and (if required) upload supporting documentation (vaccination certificate, or medical exemption). The Attestation report presents real time data for administrators to analyze the vaccination status for all employees and includes the ability to output status information for further reporting.
